Sharraxaadda

Water valve cover (on Mayfield) just west of Mayfield and Sherry Drive intersection has spun off from large vehicle traffic passing over it. Appears that cover has come off several times due to damage present on pipe (see photos). There are 2 covers located within 15 feet of each other - both becoming damaged due to failing pavement surrounding them.
