For best results you should contact the North Haven Public Works Department directly.

I'm grateful to North Haven's First Selectman for sending out the Public Works guys so quickly. But their patch really did nothing. The stretch of damaged road that makes driving into my driveway like going over a speed bump was left untouched. Is it possible they're coming back to do more? Probably not.... I've attached a picture here that shows the small bit that was done. I'll attach a close up in another comment.
