Call the Police Department and double check, but I believe that the first step is to inform Public Works of the incident--they should give you a case number--and this step needs to be taken *within* 24 hours of the incident. Then fill out the form online (search the City site under "property damage") and bring it to City Hall.

I have at least 3 pot holes on my street which is wooster st in new haven i had to buy 4 tires for my vehicle due to alignment issues there is also a massive pot hole on state st in front of the jet cleaners driveway that also is a major issue. Please het these addressed.
