It finally seems to be "fixed." The piece of pipe has been capped off with a metal cover that says "CITY WATER." The concrete is still broken around it and could offer a tripping hazard, but the situation is greatly improved. After a year and a half of flogging on this, I'm posting the photo I took today and calling it closed, although still not optimal. I have no idea why this is water issue and what's under that cover....
