Dear Sir,

I would like to report loss of my passport for which details are as below
Name : Dheeraj Kumar Ashiwal
DOB : June 8th, 1975
Number : H6636311
Country of Issuance : India

Last i remember my passport was with me when i rented one car from hertz which was Impala. I had to return that car the very next day as they gave that car to me by mistake. I earlier thought i might have left it in the car but i checked with them and they were not able to trace my passport.
After that i have moved my apartment from 701 to 741 as there was fire alarm in floor above me which flooded my apartment with water. The passport might have been lost during that movement also.

I will report this with Consulate of India today itself.

I am attaching my passport and visa related documents with this report.

Please advice if i need to provide any more documents or inform any other authority.
