I have contacted Verizon myself and put a order in to have this fixed. They gave me a confirmation number of MDCF046YNJ. They said I will be contacted within 48 hours for someone to come out. The only information I had for you is the address and your reported name. Feel free to contact Verizon back 888-836-6532 and use the reference above to update any information. I do hope this helps.
