    I previously submitted this request however the exact location in this request is more precise/accurate than in my previous request. The access panel needs a cover. There are 2 orange cones on top of the access panel and it sits near the middle of the sidewalk.

    This access panel for the lamp post has not had a cover on it for years. Two orange cones are the only things that keep people from falling in the hole. This uncovered access panel is near the driveway of the ATF building on N St near the corner of 2nd St NE. Please have it repaired.

    The signal bulb is not out but there was no other answer that was applicable. The new traffic light for vehicles on Perry Street NE at Michigan Avenue stayed red for several cycles and never turned green.

    I answered that this incident occurred at night because an answer was required but I have no way of truly knowing this because I didn't witness it. There are some tires and other debris on Puerto Rico Ave. that needs to be removed. People always dump things on this street. There needs to be more signage/lights and/surveillance of this area to prevent this from happening.

    I reported that the red light bulb was not working but the green light was working. Well now the red light has been fixed on the light to the far left but not the one in the far right. I will upload another photo.

    The light on the far left has a red, yellow and green light. The light to the far right does not have a working red light for traffic exiting O Street parking lots.

    I noticed that the red and yellow traffic lights are not working yesterday evening but for some reason I lost the submission and had to wait until I could take another picture. The traffic light in question is facing the cars exiting the O Street parking lots in front of 64 New York Ave NE. The red and yellow lights on both signals are out but the green lights work.
