    On 11th St, Westbound, corner of Washington Avenue, multiple deep potholes are a hazard. These were filled just a few weeks ago and now are back just as bad as before.

    At SE corner of Bayberry Avenue and Lefferts Road, small metal cover of the street light pole is off and lying on the ground. Needs to be re-attached because internal wiring of the pole is exposed at this point.

    Street lights in front of 6 and 14 Spruce St. are out. National Grid excavated on the same side of Spruce St. on Oct 3 and 4 and likely cut or damaged the underground electric cable for these street lights.

    Corner of Mulberry Ave. and Washington Ave. is missing the Mulberry Ave. street sign due to incident in August when a large tree branch fell and crushed the Mulberry Ave. street sign, which was then removed but not replaced.
