Multiple cones (pieces of cones) and debris in the road here. Mainly on the exit ramp from Chippenham Parkway North onto Forest Hill Ave East.

Large pothole here. It is on the right shoulder of the exit from Chippenham Parkway North onto Forest Hill Ave East. It is right as you are making the turn from north to east.

Large pot hole here on eastbound Monument ave. It is by the church.

Large pothole here. It is in the east bound right lane. It affects drivers getting off of Powhite heading east onto Forest Hill Ave.

Large pothole here as you are turning left onto Bremo Rd (towards St. Mary's Hospital) from Monument Ave West. The pothole is in the intersection and affects the drivers turning from Monument onto Bremo.

The light at Hioaks and Jahnke does not work properly. It is often green for traffic on Hioaks Road to cross over Jahnke when there is no traffic going in that direction. It stays green in this direction for a very long time while traffic on Jahnke stays backed up and no one is driving on Hioaks. I have noticed this issue at all times (even 4:30am when I am driving through) and I am the only car driving through that intersection.

I submitted a request for this issue on 9/8/2016 (Issue ID 2883660, Service Request ID 320149). This issue was marked as "VDOT" by WILLIAMS JR, JAMES, so I submitted a request to VDOT. They reported back to me almost immediately that this is a City of Richmond issue since it is within the city limits.
