Thanks so much for the quick response Karen. I don't feel comfortable giving my address. Hopefully the photo of the problem and the physical location(this was reported from my smartphone w GPS) will be enough for you guys to address the issue.
If you can't because its against policy I completely understand. Well just have to wait till someone calls into 311 w their info.
