Aluminum junction box cover has been missing for a very long time. Power cables in junction box are exposed and pose an electrical safety hazard. Request a new, original style cover be installed ASAP.

Note: Before I could submit this report all symptoms had to be address. I check yes to light's condition but do not know the lamp's condition. As result I suggest that this reporting process be changed to allow as little a one problem to finish a report.

Aluminum junction box cover was missing for a very long time. Tape as temporary solution was placed months ago. Request tape be replace with proper original style cover.

Electrical junction box cover is missing allowing easy access to energized cables. Now has a candy bar wrapper stuffed in the hole. Halloween trick or treaters may do the same. Consider this as personal safety risk and hazard.

Fire hydrant has a missing/damaged mounting clamp. One of the clamps segments appears to corroded away. This is the second I have discovered along this section of Michaux Drive.

Light comes on then goes out. Repeat.

Overgrown Bush/Tree on private property is obstructing side walk on Whipaway Lane tangent to the rear property line of 978 Kelso Ct and 981 Edwin Dr.

Issue ID: 1623345, VBWorks number is 1623345 was closed and archived. Problem not corrected. Resubmitting.

Location: 4900-4924 Providence Road at the north west corner of Providence Road and Edwin Drive intersection. Ponding occurs at this corner because the run off path is blocked by dirt and debris. Ponding can reach into the intersection covering the right turn lane stop bar.

Open Verizon Pedestal; Potential Shock Hazard. Pedestal has been open for months. Quote from:

To ring your telephone, the phone company momentarily applies a 90 VRMS, 20 Hz AC signal to the line. Even with a thousand ohms of line resistance, this can still pack a bit of a shock so be careful when you are probing around trying to find a POTS line.

Light Pole Electrical Shock Hazard: The electrical cable junction cavity at the base of the light pole is exposed with the electrical cables extending out of the pole's recess making it very easy to grab the energized cables. Cover has been at the base of the pole for greater than a year and has been previously report to Dominion with no action taken.

The drainage swell that parallels the eastern boundary of the Providence Elementary and Kempsville Middle School properties has filled in and does not drain storm water run off. Water remains in many areas after rain stops. Mowing is not done in wet areas because the ground stays muddy. Mosqitos proliferate and pester students and sport spectators. The swell has filled in primarily from ball diamond resurfacing and poor grading/silt control during the air conditioning upgrade to Providence Elementary. Attached is an annotated picture which shows the swell location and water flow path.
