Description

On Tuesday, May 28, 2013 at approximately 7:00 a.m. I hit a gaping pothole about 100 to 200 feet before the first left entrance of the Air Rights garage on North Frontage Road (MLK Blvd. now). My attempt to avoid it was limited due to the traffic immediately beside me on my right side. My front left tire hit a portion of the pothole and it caused a flat tire that resulted in me taking time off work to pay for a $158.74 repair. I have attached a picture of my damaged tire that I am seeking reimbursement for. Additionally, I have the receipt for the damage from the repair shop but I can only attach one item on this form.
