I received a phone call from a City Inspector who was assigned to investigate this problem. Unfortunately he was unable to locate the broken water meter cover. I provided him with all of the information you submitted, but he still was unable to find the hazard.

@CAO Fisher - Thank you for your attention to this matter. We have hit another roadblock however.
As management of the park spaces in Mission Bay, and adjacent to this water meter cover that is broken, I provided this Inspector's phone number to my Site Engineer who had been doing some work on finding the correct entity to address the issue as well. He called the inspector whose number you provided. The inspector was rudely terse and said that he was not the person to be calling on this matter.

Maybe post a photo of the box with the missing cover.
