To Whom It May Concern:

The purpose of this e-mail is to inform/report an accident that happened on November 29, 2015 around 2:00pm. This took place on Tampa Road, across from Windwar Place as you can see on the pictures I have attached.

I was finishing my afternoon walk when I tripped on a bolt that was not properly secured in the middle of the sidewalk on the “Traffic Light” cover as shown in the attachments as well. Due to this accident caused by the bolt sticking out, I plummeted downward, hit my face, my right knee, as well as my left arm from the shoulder down to my wrist. This could have been very serious as it is extremely close to a busy road with oncoming traffic.

Immediately, my arm and elbow began to swell up and I was in a lot of pain. I waited until the evening to see if things would get better but as the time went by, the swelling kept going up. I contacted the City of Oldsmar yesterday afternoon as I thought this location fell under their jurisdiction and I was redirected to contact Pinellas County this morning.

I went to the hospital last night to get an X-ray done because I could not stand the pain anymore and I did not want to wait any longer to see what had happened. Thankfully, I did not break any bones but I was put in a sling and referred to a bone specialist to see if there is a possible fracture in my elbow next week since as of right now the skin is swollen and the Dr. at the emergency room told me it is possible it can be fractured but it can take a week for the swelling to go down and for them to be able to see a fracture in the bone.

My insurance does not cover emergency visits until I have reached my deductible, which is ridiculously high. I do not have the financial conditions to pay for my hospital visit last night as I was informed I am responsible 100% for the charges and my insurance will not be covering it. I was not expecting to have this expense placed on my hand when I set out for my walk yesterday afternoon. As I mentioned, I cannot afford to pay my hospital visit and it is recorded there that I am waiting to hear back from the City to see if you are able to help me with this expense.

This accident was a result of negligence/carelessness. Whomever worked on the Traffic Light cover could have taken an extra 3 seconds to make sure the bolt was secured down into the ground, but instead they left it sticking out. This bolts needs to be fixed ASAP to avoid any other accidents from happening. The bolt clearly shows that it was not secured properly last time it was placed into the ground when this area was last serviced.

Thank you for your prompt attention in regards to this matter and I hope I can count on my city to help me pay my hospital bill as I am financially unable to.
