AN ASSESSMENT WAS MADE AT THE REQUESTED LOCATION ON THE DATE OF 5/5/2016. THERE WAS A SMALL SINKHOLE LOCATED APPROXIMATELY 10 LINEAR FEET FROM THE DRIVEWAY OF THE REQUESTED LOCATION TO THE ROADWAY. THE UTILITIES NOTICED IN THE VICINITY OF THE SINKHOLE WAS A SEWER MAIN LINE. THIS REQUEST WILL BE CLOSED AND A REQUEST WILL BE OPENED IN CRS FOR FURTHER INVESTIGATION BY THE SEWER DIVISION.
