The 2-Way cleanout at 2967 Madeline St. required by OMC 13.08.400 was removed by a PSL contractor on 4-6-12. See Google Maps for a photo of what the cleanout looked like before it was removed. The cleanout was located in the planting strip as are the rest of the cleanouts on the block (installed by
andes construction as part of a neighborhood sewer rehab project) If you scroll the Google map photo at the attached link you can see the cleanout next to the utility pole. The cleanout is no longer there.

And here is a photo taken after the 2-way cleanout was removed. The patch in the street has sunk four inches in the last 10 days. Did this repair have a COMPACTION TEST?
